Wellness Starter Plan
1 home visit/month
Ideal for: Independent adults or seniors who want to stay on top of their health with preventative support and wellness check-ins.
Includes:
• One monthly in-home wellness visit (60 min)
• Vital signs, wellness assessment
• Medication review and reminders
• Basic health education and coaching
• Monthly goal tracking sheet
• Coordination with PCP (if needed)
Chronic Care Support Plan
2 home visits/month
Ideal for: Individuals managing one or more chronic conditions who need more frequent monitoring and health education.
Includes everything in the Wellness Starter Plan, plus:
• Bi-weekly in-home visits
• Chronic condition monitoring (e.g., blood sugar, blood pressure, weight)
• Functional assessment (ADLs)
• Red-flag symptom tracking and escalation
• Medication setup (pill box prep, calendar)
• Nutrition and hydration guidance
Post-Hospital Recovery Plan
4 home visits/month
Ideal for: Anyone recently discharged from a hospital or rehab facility who needs support recovering safely and avoiding re-hospitalization.
Includes everything in the Chronic Care Support Plan, plus:
• Weekly in-home visits
• Wound care monitoring and light dressing support
• Medication reconciliation and updates
• Discharge plan review and support
• Mobility and movement education
• Progress tracking and documentation for providers
Comprehensive Care Plan
6 home visits/month
Ideal for: Clients or families needing robust wellness oversight, frequent visits, and full care coordination.
Includes everything in the Post-Hospital Recovery Plan, plus:
• 1–2 visits weekly (based on client needs)
• Personalized wellness coaching (nutrition, sleep, lifestyle)
• Custom health routines and habit support
• Family/caregiver monthly wellness update
• Care coordination with multiple providers/family members
• Ongoing documentation and reassessment of needs
